GPF Handler
 MrDebug records and informs on GPF's

 When the GPF occurs MrDebug logs the GPF details to a .GPF file. If the
 program is stable enough to continue, then MrDebug scans the .MAP file
 updates the .GPF file with Function name and modules name with MAP A,S and
 then outputs the results to the Error/GPF window.
